Offers the following programs:
Human Services Center has two Transitional Living facilities. One is a six bed home that requires minimal supervision. Single adults may live there on a long or short term basis, depending on their need. The other facility is a three bed home that requires daily supervision. This home is for persons assimilating into less structured housing.
LONG TERM AFFORDABLE HOUSING
Currently, HSC has 56 beds available for person with housing needs. This includes 42 apartments. Housing is primarily for single individuals.
HSC provides staff assistance to clients who are interested accessing housing and other related services. A caseworker works with clients to identify community-housing opportunities and to prepare them for independent living.
EDGEWOOD (LTSR) LONG TERM STRUCTURED RESIDENCE
Edgewood is an eight bed state of the art facility that provides comprehensive and intensive mental health services to its residents. Therapeutic objectives are designed to be accomplished in 30 to 180 days. Nursing staff are on duty 24 hours a day. A master’s level clinician is on site 8 hours per day. This facility is designed to meet the needs of the more challenged individual who often had a history of multiple hospitalizations or would otherwise experience long-term hospitalization.
Westfield is dually licensed as a personal care home. This eight bed facility is located on a farm setting in a rural area. All residents are involved in the on-site vocational programs. This program is designed for persons that have spent time in a state mental institution or have a long history of mental illness.
HSC has three Fairweather Lodge facilities where we provide long-term housing in a group setting. Almost all residents have their own room Staff is available for support as needed. Residents develop and establish house rules and are involved in the selection of new residents. Positive peer support is integral to this successful program.
COMMUNITY-HOSPITAL INTEGRATION PROGRAM (CHIP)
The following program is part of the Center’s CHIP which provides housing and supportive services to individuals who have been discharged from a state institution or who have a history of mental illness.
Horizon House is a dually licensed Personal Care Home/CHIP facility which serves as an entry point for community placement. It provides housing and intensive day treatment programming for up to eight clients. The focus of Horizon House is on meeting the needs of clients with an extended history of mental illness.
HSC’s housing units are licensed by the Commonwealth of Pennsylvania . HUD also ensures that housing developed with their funds conform to federal standards for decent, safe, and sanitary housing.
HSC’s housing initiatives are closely integrated with the clinical services offered by the Center. Clinical staff members are aware of the housing options and services offered by this department, and, when appropriate, are included in the client’s service plan.
